1. When modifying a form, click on the text field and write the instructions for your task. After doing so, click on the ”Multiple choice questionnaire” button from the form field menu. You will see a text that reads:

“What is your favorite color?
[[o:s=0]] Red
[[o:s=0]] Blue
[[o:s=0]] Yellow”.

2. Add your own question, answers and point values:

• Replace the example question with your own question and the example answers with your own answers.
• Replace the number value that follows “s=” with the point value for the answer.

3. You can examine the participants’ answers individually or as a summary by clicking the “Responses” button.
